The PAEC Medical Module allows nurses and contracted Health Department staff to enter School Health Services and Screenings in FOCUS as well as pull the EARS report as needed to report this information in the Florida Department of Health HMS (Health Management System).
The purpose of this article is to familiarize school health personnel with recording Clinic visits in the School Health Services log.
If you have questions regarding this module, please consult your District Health Coordinator. Your District Health Coordinator will work with the MIS System Administrator if necessary.
Recording School Health Services (Visits to the Clinic)
School Health Services are logged under the student's demographic information on the Health Services Log tab. Here, school health staff may record and review health interaction logs with students that usually come from an unscheduled clinic visit.
School Health Services Log Field Descriptions

| Time In | Time that student entered the clinic |
| Referral Source | If the student was sent to the clinic by another party, the applicable option should be entered here |
| Referral Category | Category of the student's complaint or reason for being sent to the clinic |
| Intervention Administered | Appropriate health service code from the FLDOH School Health Services manual. If you are looking for a service that is not in the list, please consult your District Health Coordinator. |
| Intervention Administered By | School health staff member that administered the intervention |
| Intervention Outcome/Disposition | Student outcome after the intervention was completed |
| Time Out | Time that student left the clinic |
| Referred To | If the student was referred for further medical treatment, the applicable option should be entered here |

Recording Entries in the School Health Services Log

Option 1: Using the Mass Add Log Records Screen (Recommended)
To record School Health Service Log entries on a single screen in FOCUS that saves to each individual student, complete the following steps:
- In FOCUS, click Students->Mass Add Log Records
- In the "Logging Field" option, search for "School Health" and select "School Health Services Log"
- On the "By Student" tab, select the desired student in the first column of the first row to record information pertaining to the visit. When finished, press "Enter" on your keyboard or click "Save" to save the record. The entry will be moved to row 2 and a new blank line will appear at the top in preparation for the next entry.
Nurses often prefer to utilize this method for faster entry and the ability to record visit information without navigating individual student screens.
Option 2: Using the Student Demographic Screen
To record School Health Service Log entries individually by student, complete the following steps:
- In FOCUS, search for the desired student
- Under the Demographics tab, click "Health Services Log"
- Add the respective information related to the clinic visit at the top of this field. When finished, press "Enter" on your keyboard or click "Save" to save the record. The entry will be moved to row 2 and a new blank line will appear at the top in preparation for the next entry.
