Discipline data is reported every year in Surveys 2, 3, 5 and monthly in Survey 5. Referrals are reported on the Discipline Resultant/Action format if any state defined action code is used (see section Adding Actions above for a list of these codes). All SESIRs are reported on the School Environmental Safety Incident Report format.
Certain fields have specific relationships to other fields, and if you do not properly assign their codes they can result in errors, see below for a list of some of the most common errors we see:
- If the referral Use of Drugs code is Y, then there must be a Drug Description on the linked SESIR
- If the student is being expelled you must indicate if the expulsion is related to zero-tolerance or not in the Zero-Tolerance: Expulsions code
- If the referral Weapon Use code is Y or SESIR Discipline/Offense Code is Weapon Possession, then there must be a Weapon Code on the linked SESIR
- If the SESIR Discipline/Offense Code is Hazing, then Hazing Related field must Yes
- If the SESIR Discipline/Offense Code is Homicide, then Victims of Homicide must be filled in.
- If the SESIR Discipline/Offense Code is Alcohol Possession then Alcohol Related field must Yes
- If the SESIR Discipline/Offense Code is Drug Sale/Distribution or Drug Use/Possession then Drug Related field must Yes
- If the SESIR Discipline/Offense Code is Battery or Sexual Battery then Injury Related field must Yes
- If the SESIR Discipline/Offense Code is Bullying, then Bullying Related field must Yes
- If the SESIR Discipline/Offense Code is Battery, Homicide, Kidnapping, Sexual Battery or Weapon Possession then Reported to Law Enforcement must equal Y
- If the SESIR Involvement Type is Student or Student and Non-Student there must be at least one linked Student referral record with a SESIR defined action.
- If the Use of Drugs code on referral is Y, then the Drug Description code must be assigned.
Note: some of the errors listed on the DOE Validation reports are labeled as Exceptions, when you see exception errors it is important to review them but note this type of error will not cause the records to reject and is to serve as more of a warning.
